Nursing home cost: a private resident room in a skilled nursing facility with an adjustable bed, wheelchair and window light

A private room in a skilled nursing facility, the most expensive way to receive long-term care.

Three midnights. That is how long a patient must be formally admitted to a hospital before Medicare will pay for a single day of nursing home care, and families who miss that detail can owe the full rate from the first morning. That rate is steep: a national median of $315 a day for a semi-private room and $355 for a private one, or $114,975 and $129,575 a year, according to CareScout’s 2025 Cost of Care Survey, the most recent national figures available for 2027 planning.

So the useful answer to “how much does a nursing home cost” is a schedule, not a single number. The bill changes hands several times: from Medicare, to a daily copay, to the family’s savings, and for most long-stay residents, eventually to Medicaid. Each handoff has a date you can predict and a mistake you can avoid.

The schedule at a glance. After a qualifying hospital stay, Original Medicare pays in full for days 1 to 20 of skilled care. Days 21 to 100 cost $217 a day in 2026, and the Medicare Trustees project $223.50 for 2027 (CMS had not published the official 2027 figure when we checked in early November 2026). From day 101, or sooner if skilled care ends, the resident pays the full rate until savings run low enough to qualify for Medicaid.

Before the transfer: the hospital stay that decides who pays

Medicare’s nursing home benefit only switches on after a qualifying inpatient hospital stay of at least three days, counted as three midnights as a formally admitted inpatient. The day of discharge does not count. Neither does time spent under “observation,” which is billed as outpatient care even when the patient sleeps in a regular hospital bed for days.

This is the single most expensive misunderstanding in the whole timeline. A family that assumes Medicare will cover rehab, only to learn the hospital stay was coded as observation, can owe the facility’s full private rate from the first day. At the national median, three weeks of that is roughly $6,600.

  1. Ask the attending doctor or case manager, in plain words, “Is she admitted as an inpatient, and since what date?”
  2. Hospitals must give Medicare patients held in observation for more than 24 hours a written notice, the MOON form. If you receive one, the three-day clock is not running.
  3. If the patient is on a Medicare Advantage plan, ask the plan directly. Many Advantage plans waive the three-day rule but require prior authorization for the facility instead.

Days 1 to 20: Medicare’s full-coverage stretch, with conditions

With a qualifying stay behind you and a doctor’s order for daily skilled care, Original Medicare covers the first 20 days at $0 beyond the Part A hospital deductible already paid ($1,736 per benefit period in 2026). That covers a semi-private room, meals, nursing, therapy, medications given in the facility and medical supplies.

The condition that matters is the word skilled. Medicare pays for rehab and nursing that need licensed staff, such as physical therapy after a hip repair, IV medications or wound care. It does not pay for custodial care, meaning help with bathing, dressing, eating and getting to the bathroom, when that is all a person needs. That distinction drives everything that follows.

Medicare pays for

  • Short-term rehab after a hospital stay
  • Daily skilled nursing or therapy
  • Up to 100 days per benefit period

Medicare never pays for

  • Long-term custodial care
  • A private room unless medically necessary
  • Any day once skilled care is no longer needed

Days 21 to 100: a $217 daily copay, and what 2027 adds

From day 21, the resident owes coinsurance for each day: $217 in 2026. If someone used all 80 of those days, that comes to $17,360. The 2026 Medicare Trustees Report projects $223.50 a day for 2027, which would make the same 80 days $17,880. CMS normally publishes the official number in November, so check Medicare.gov before you budget to the dollar.

Who pays the copay depends on the coverage behind Medicare. Most Medigap plans, including the popular Plan G and Plan N, cover skilled nursing coinsurance in full. Medicare Advantage plans set their own daily copays and approval rules, so the plan’s Evidence of Coverage is the document to read, not the brochure. Retirees with Medicaid as well as Medicare usually owe nothing.

Two facts families rarely hear. First, 100 days is a ceiling, not a promise: coverage stops whenever the resident no longer needs daily skilled care, and the facility must give written notice before Medicare coverage ends. You can request a fast appeal through the Quality Improvement Organization named on that notice, and it costs nothing. Second, Medicare does not require a patient to keep improving. Under the 2013 Jimmo v. Sebelius settlement, skilled care needed to maintain a condition or slow decline can still qualify, so “she has plateaued” is not by itself a valid reason to end coverage.

Month four and beyond: the private-pay rate, in full

Once Medicare drops off, the facility bills its daily rate. CareScout’s 2025 medians rose just 2% for semi-private rooms and 1% for private rooms, a calmer year than the jumps CareScout saw before it. Over the stretches families actually face, those rates add up fast:

Length of private-pay staySemi-private room ($315/day)Private room ($355/day)
One month$9,581$10,798
Six months$57,488$64,788
One year$114,975$129,575
Two years$229,950$259,150
Three years$344,925$388,725
National medians from CareScout’s 2025 Cost of Care Survey, held flat. Real bills usually rise each year and vary widely by state.

Location moves these numbers more than anything else. CareScout ranked Alaska, Oregon, New York, Hawaii and Connecticut as the most expensive states for a semi-private room in 2025, and Texas, Missouri, Oklahoma, Arkansas and Louisiana as the cheapest. A facility 30 miles away, across a county or state line, can be meaningfully cheaper, which is worth weighing against the cost of visits.

The daily rate is also not the whole bill. Ask for the facility’s list of charges outside the basic rate. Pharmacy, personal laundry, phone and cable, beauty and barber services, and some supplies are often billed separately, and a private room on Medicaid typically means paying the difference yourself.

The admission packet: one signature to refuse

Admission agreements are long and often handed over in a rush. Read the signature lines closely. Federal regulations (42 CFR 483.15(a)(3)) bar Medicare- and Medicaid-certified nursing homes from requiring or even requesting a third-party guarantee of payment as a condition of admission. An adult child can sign as the resident’s representative, which obligates them to use the resident’s money to pay, but not as a “responsible party” who owes the bill personally. If the paperwork blurs that line, cross it out or ask the state long-term care ombudsman to review it before signing.

While you are reading, look for any binding arbitration clause. Federal rules say it cannot be a condition of admission, so you can decline it.

The spend-down: when savings meet Medicaid

Medicaid is how most long-stay residents end up paying; KFF reports it is the primary payer for roughly six in ten nursing home residents. Eligibility is set by each state, but many use a countable-asset limit for a single applicant of about $2,000, matching the federal SSI standard. Once approved, the resident’s income, minus a small personal needs allowance and health insurance premiums, goes to the facility each month, and Medicaid covers the rest.

Three federal rules shape how fast a family gets there:

Rule2026 figureWhat it means for the timeline
Five-year look-back60 monthsGifts or below-value transfers in that window trigger a penalty period with no Medicaid coverage.
Community spouse resource allowance$32,532 to $162,660A spouse still at home keeps assets in this range, depending on the state.
Spouse’s monthly income allowance$2,643.75 to $4,066.50The at-home spouse can receive part of the resident’s income to reach this level (higher minimums in Alaska and Hawaii).
Home equity limit$752,000 to $1,130,000A single applicant with more home equity than the state limit is ineligible for long-term care coverage.
Spousal and home equity figures from the CMS bulletin of December 9, 2025. They are indexed to inflation and update each January.

One change is already on the calendar. Under the 2025 budget reconciliation law (H.R. 1), starting January 1, 2028, federal law caps the home equity limit at a flat $1,000,000, not indexed to inflation, according to an analysis by Justice in Aging. States now using the higher $1,130,000 option will have to come down. The cap does not apply when a spouse or certain children live in the home, and homes on agriculturally zoned lots keep the older rules. For 2027 itself, the existing limits continue with their normal inflation update.

One stay, start to finish, with a $150,000 nest egg

Hypothetical example for illustration. A widowed 82-year-old with $150,000 in savings and $2,100 a month in Social Security and pension income has a qualifying three-day hospital stay, goes to rehab, and is not able to return home. She has Original Medicare without a Medigap plan, and the facility charges the national median for a semi-private room.

  • Days 1 to 20: $0 to the facility.
  • Days 21 to 100: 80 days at $217 is $17,360, leaving about $132,640.
  • Month four onward: the $9,581 monthly rate minus her $2,100 income is a $7,481 monthly draw on savings.
  • About 17 months later: savings fall to roughly $2,000 and she applies for Medicaid, roughly 20 months after admission.
  • After approval: nearly all of her $2,100 still goes to the facility each month; Medicaid pays the remainder.

Change one variable and the date moves a lot. A Medigap plan would have saved the $17,360 copay and added more than two months before the spend-down ends. A private room would shorten the runway by about two and a half months. A married couple would protect far more for the spouse at home, which is why couples have the strongest reason to talk to an elder law attorney early.

Insurance and taxes that soften the middle years

Long-term care insurance, if someone bought it years ago, usually pays a daily or monthly benefit after an elimination period, often 90 days, which conveniently overlaps the Medicare window. Pull the policy now and check the daily maximum against the $315 to $355 medians; older policies without inflation riders often fall well short.

Taxes help more than people expect. Nursing home costs are deductible as medical expenses when the main reason for being there is medical care, under IRS Publication 502, to the extent total medical costs exceed 7.5% of adjusted gross income. For people still paying premiums, the 2026 caps on deductible long-term care insurance premiums run from $500 at age 40 or younger to $4,960 at 61 to 70 and $6,200 over 70. A tax professional can confirm what applies to your return.

Questions families ask once the bills start

Can a nursing home discharge a resident when the money runs out?

Federal rules allow discharge for nonpayment only after reasonable notice, generally 30 days in writing, and a resident whose Medicaid application is pending is not treated as unpaid until the claim is denied. Apply for Medicaid before savings are fully gone and give the facility copies of the paperwork. The state long-term care ombudsman can help if a discharge notice arrives.

Does Medicaid take the house after a nursing home stay?

Not while the resident or a protected family member is alive and the home qualifies as exempt. After death, federal law requires states to seek estate recovery for long-term care costs, which can include the home, though recovery is deferred while a surviving spouse or certain children are living. Rules vary by state, so confirm with an elder law attorney.

Does the VA pay for nursing home care?

For some veterans, yes. The VA provides nursing home care through its Community Living Centers, state veterans homes and contracted community facilities, with cost depending on service-connected disability rating and income. Ask the VA medical center’s social work office before admission to a private facility.

Is memory care in a nursing home priced differently?

Often. Many facilities charge more for a secured dementia unit than for a standard semi-private room, and a few bill behavioral care as an add-on. Ask for the written rate for the specific unit, and compare it with assisted living memory care, which can cost less for people who do not need skilled nursing.

Are nursing home rates negotiable?

Daily rates for long-term residents are rarely discounted, but there is room at the edges. Families sometimes get a lower rate for a semi-private room by agreeing to a roommate, or avoid add-on charges by supplying their own laundry or personal items. Getting written rates from three facilities is the best leverage.

Find your day on the calendar, then make one move

  • Still in the hospital: get the inpatient admission date in writing, and ask the case manager which nearby facilities take Medicaid as well as Medicare. A Medicaid-certified bed now can spare a move later.
  • Somewhere in days 1 to 100: have your State Health Insurance Assistance Program (SHIP), a free Medicare counseling service, check what the Medigap or Advantage plan owes on the daily copay, and appeal any coverage-ending notice you disagree with.
  • Private pay with savings still well above $2,000: book an elder law attorney now, while there is something left to plan with, especially if a spouse lives at home.

Pricing method: prices were researched in November 2026 for 2027 planning and may change. National nursing home medians come from CareScout’s 2025 Cost of Care Survey (released March 2026); Medicare figures from Medicare.gov and the 2026 Medicare Trustees Report projections; Medicaid spousal and home equity standards from CMS’s December 2025 bulletin; the 2028 home equity cap from Justice in Aging’s analysis of H.R. 1; tax limits from ElderLawAnswers’ 2026 roundup. Eligibility rules vary by state, and this article is general information, not legal, tax or financial advice.
