Hospice cost: family member holding an elderly patient's hand beside a hospital bed at home

Most hospice care happens at home, where Medicare covers the care but not the caregiving hours.

Your mother’s oncologist says the word hospice on a Tuesday, and by Thursday a nurse is sitting at the kitchen table with a folder of forms. Somewhere between the medication list and the election statement, the family starts wondering the thing nobody says out loud: what is this going to cost us? If she has Medicare and stays at home, the hospice care itself costs nothing, and the out-of-pocket bills are usually limited to drug copays of up to $5 and a few dollars a day for respite stays.

That answer changes fast with the setting. Move the same patient into a nursing home and the family may owe about $9,450 a month for the room, because Medicare’s hospice benefit does not pay room and board anywhere. Under 65 with no coverage, a hospice could bill several thousand dollars a month. Find the row below that matches your family, then read only the section you need.

Start with where care will happen and who is insured

Your situationWhat the family typically paysWho covers the rest
Original Medicare, care at homeUp to $5 per prescription; 5% of respite stays; any paid caregiver hoursMedicare Part A
Medicare, living in a nursing homeRoom and board, a national median of $315 a day for a shared roomMedicare pays the hospice care only
Medicare, living in assisted livingThe community’s rent and care fees, a median $6,200 a monthMedicare pays the hospice care only
Medicaid, or Medicare plus MedicaidUsually little or nothing; income may go toward the nursing homeMedicaid, including nursing home room and board
Enrolled veteranNo copays for hospice through the VAVA health care
Under 65, private insurance or nonePlan deductibles and coinsurance, or the full daily rateYour plan, if it covers hospice
Room and board and assisted living figures are CareScout 2025 national medians; your state may be well above or below them.

For scale: Medicare paid hospices about $30.3 billion in fiscal 2025 for 1.92 million patients, according to the CMS Hospice Monitoring Report published in April 2026. That works out to roughly $15,750 per patient, our arithmetic, almost none of which families see. Hospice is now where 53.8% of Medicare decedents spend their final days.

At home on Original Medicare: tiny copays, unpaid hours

Medicare.gov is plain about this: you pay nothing for hospice care from a Medicare-approved provider. That covers the nurse and doctor visits, the aide who helps with bathing, social work, chaplain visits, medical equipment such as a hospital bed and oxygen, and supplies related to the terminal diagnosis. Two small charges remain:

  • Prescriptions: up to $5 per prescription for outpatient drugs that control pain and symptoms.
  • Respite care: when the family caregiver needs a break, Medicare covers an inpatient stay of up to five days at a time, and the patient owes 5% of the Medicare-approved amount. At the fiscal 2027 respite rate of $545.93 a day, that is about $27 a day, or roughly $137 for a five-day stay. The total is capped at the Part A inpatient deductible, which was $1,736 in 2026.

The expense families underestimate is time. Routine hospice at home means scheduled visits, not a staffed shift. The hospice aide may come two or three times a week for an hour, and the nurse is on call overnight by phone. Everything in between falls to family, or to paid help that Medicare does not cover. CareScout’s 2025 survey puts a non-medical caregiver at a national median of $35 an hour. Forty hours a week of private help is about $6,070 a month; round-the-clock coverage at that rate passes $25,000.

That is why many families mix the two: hospice for the medical side, a few hired shifts for nights, and relatives for the rest. Our in-home care cost guide breaks down agency and private-hire rates by weekly hours.

Living in a nursing home: hospice arrives, the room bill stays

Hospice can be delivered in a nursing home, and the hospice team will visit just as it would at a house. What Medicare will not do is pay the facility for the bed and meals. CareScout’s 2025 national median for a semi-private room is $315 a day, or $114,975 a year; a private room is $355 a day. In a 30-day month that is about $9,450 or $10,650.

The trap here is timing. A patient who entered the nursing home for rehab after a hospital stay may have had Medicare paying under the skilled nursing benefit. Electing hospice for the same terminal condition generally ends that skilled coverage for that condition, and the family can wake up to a private-pay bill. Before signing the election statement, ask the nursing home’s business office one question: who pays for the room starting tomorrow? Our nursing home cost guide walks through daily rates and the path to Medicaid.

Assisted living: the rent keeps running

For residents of assisted living, the arithmetic is simpler but no smaller. Hospice comes in as a visiting service, Medicare pays the hospice, and the resident keeps paying the community. CareScout’s national median was $6,200 a month in 2025, up 5% in a year.

Watch the care-level fees. As needs rise near the end of life, many communities move residents to a higher care tier, which can add hundreds or more a month even while hospice aides are doing part of the hands-on work. Some contracts also require residents who need two-person transfers or round-the-clock nursing to move out. Read the discharge clause before a crisis forces the question. Our assisted living cost guide covers how those tiers are priced.

On Medicaid, or on both Medicare and Medicaid

Medicaid is the program that closes the room-and-board gap. Nearly every state Medicaid program covers hospice, and for a Medicaid-eligible patient living in a nursing facility, federal law has Medicaid pay the hospice an amount equal to at least 95% of the state’s nursing facility rate, which the hospice then passes to the facility. For dual-eligible patients, Medicare pays the hospice care and Medicaid handles the room.

Two caveats. The patient’s income usually still goes toward the cost of the nursing home under the state’s share-of-cost rules, minus a small personal needs allowance. And Medicaid eligibility takes time to approve, so a family applying in the final weeks may carry private-pay bills until approval, which many states make retroactive for up to three months. A state Medicaid office or an elder law attorney can tell you how your state handles it.

Veterans: no hospice copays through the VA

The VA’s geriatrics page, updated in March 2026, states that there are no copays for hospice care, whether the VA provides it or a contracted community hospice does. It is part of the standard medical benefits package for enrolled veterans who meet the clinical need. Veterans with both VA and Medicare can generally choose which benefit to use; the hospice’s intake team or the VA social worker can sort out which one makes more sense for room-and-board questions in a community setting.

Under 65 or uninsured: what a daily rate bill looks like

Many employer and marketplace plans cover hospice, though deductibles and coinsurance apply and some require prior approval. If you have no coverage, hospices set their own private-pay rates. Medicare’s fiscal 2027 per-day rates, finalized in July 2026 with a 2.3% increase and effective October 1, 2026, are the most useful public benchmark, because many hospices quote private-pay and insurer rates in the same range. These are national base rates before local wage adjustments.

Level of care (FY2027 Medicare national rate)Per day30-day equivalent
Routine home care, days 1 to 60$236.33about $7,090
Routine home care, day 61 on$186.33about $5,590
Continuous home care (crisis periods)$1,726.16short-term only
Inpatient respite care$545.93up to 5 days at a time
General inpatient care$1,231.51short-term only

Medicare also limits how much it pays any one hospice per patient on average over a year, the aggregate cap, set at $36,174.75 for fiscal 2027. That cap is a hospice’s problem, not a family’s, but it explains why long hospice stays get extra scrutiny and why some patients who stabilize are discharged.

Five ways a family ends up with a hospice-era bill

  1. Calling 911 before calling hospice. Medicare does not cover hospital care or ambulance trips for the terminal illness unless the hospice team arranges them. Put the hospice’s 24-hour number on the refrigerator.
  2. Keeping curative treatment. Choosing hospice means giving up treatment meant to cure the terminal illness. Continuing chemotherapy or a curative drug on the side can mean paying for it.
  3. Drugs the hospice calls unrelated. A hospice may decide a heart or diabetes medication is unrelated to the terminal diagnosis. Those drugs then go back to Part D or your plan, with normal copays.
  4. Revoking and returning. A patient can leave hospice to resume treatment, and regular Medicare coverage picks back up, with its usual deductibles and coinsurance.
  5. A live discharge. In fiscal 2025, 19.1% of hospice discharges were patients still living, often because they no longer met the six-month prognosis. Once off hospice, equipment and supplies may need to be rented or bought.
How to get the list of what your hospice will not cover

Medicare lets patients request a written list of items, services and drugs the hospice considers unrelated to the terminal illness, with the reasons, and the hospice must provide it within three to five days. Starting with the fiscal 2027 rule CMS finalized in July 2026, hospices must hand that election statement addendum to every Medicare patient at election rather than only on request, so ask for it at the first meeting if no one offers it.

Hospice money questions families ask in week two

Does hospice cost money after the first six months?

No, not because of the six-month mark itself. Medicare covers two 90-day benefit periods followed by an unlimited number of 60-day periods, as long as the hospice doctor recertifies that the patient is terminally ill. Recertification after six months requires a face-to-face visit with a hospice doctor or nurse practitioner.

Can we switch hospice companies if we are unhappy?

Yes. Medicare lets a patient change hospice providers once in each benefit period without losing coverage. Families concerned about quality can compare providers on Medicare’s Care Compare site.

Does Medicare Advantage change what hospice care costs?

For hospice itself, generally no. Once a Medicare Advantage member elects hospice, Original Medicare pays the hospice. The Advantage plan can still cover care unrelated to the terminal illness and any extra benefits it offers, under the plan’s own copays.

Does hospice pay for the funeral or cremation?

No. Medicare’s hospice benefit covers care for the patient and grief and loss counseling for the family, but not funeral, burial or cremation costs, which the family or estate pays.

How much is hospice care in a residential hospice house?

Short stays for symptom control at the general inpatient level are covered by Medicare. If a patient lives in a hospice residence because care at home is no longer possible, room and board is not covered, and each residence sets its own daily rate, so ask for it in writing.

The short version for your family

Hospice care is close to free for anyone on Medicare, Medicaid or VA benefits; what costs money is the place a patient lives and the hours of care between visits. If the patient is at home with family willing to cover those hours, expect a few hundred dollars over the whole stay. If the patient lives in a facility, budget for the room as if hospice did not exist, and start a Medicaid application early if savings will not last. Either way, get the non-covered list in writing before the election statement is signed.

Pricing method: prices were researched in November 2026 for 2027 planning and may change. Figures come from Medicare.gov, the CMS fiscal 2027 hospice final rule (via Forvis Mazars and ASPR), the CMS Hospice Monitoring Report (April 2026), the CareScout 2025 Cost of Care Survey and the VA. Coverage rules vary by plan and state; confirm details with the hospice, your insurer, your state Medicaid office or a licensed professional.
