You booked a consult about the two lines between your eyebrows, mentioned the forehead while you were there, and left with a printed quote for $960. That number is not unusual. Most US practices charge $10 to $15 per unit of Botox, according to RealSelf, and CareCredit puts the national average treatment at $445, with a typical range of $325 to $710. Big-city clinics can charge $18 to $35 a unit.
The spread between $445 and $960 is almost entirely about how many units you are buying, not who is selling them. So rather than repeat a national range, we took one realistic upper-face quote and pulled it apart, line by line, the way you should before you lie back in the chair. The quote below is a hypothetical example built from published label doses and a common per-unit rate, not a bill from any real clinic.
| Line item | Units | At $15 a unit | Worth asking |
|---|---|---|---|
| Frown lines (glabellar) | 20 | $300 | Is this the label dose or a starting dose? |
| Forehead lines | 20 | $300 | Was it priced per unit or as a flat area fee? |
| Crow’s feet, both sides | 24 | $360 | Is the two-week touch-up included? |
| Total for one visit | 64 | $960 | Which brand, and can I see the vial? |
| Same plan, three or four times a year | 192 to 256 | $2,880 to $3,840 | Is there a membership rate? |
Line one: 64 units is the label’s full upper face
Botox is sold by the unit, and the FDA-approved label for Botox Cosmetic spells out the standard doses. Most injectors use these as a starting point and adjust for muscle strength, which is why men and people with very expressive faces often need more.
- Frown lines: 20 units, spread over five injection points.
- Crow’s feet: 24 units, 12 per side.
- Forehead plus frown lines: 40 units together. The label pairs forehead lines with frown lines rather than treating the forehead on its own.
- All three areas at once: 64 units, the label’s dose for a combined upper-face session.
- Neck (platysma) bands: 26 to 36 units, an indication added to the label in October 2024.
Seen that way, our $960 quote is not padded. It is the full label dose for three areas. A cheaper quote for the same three areas is usually cheaper because it uses fewer units, which can be the right call for fine lines and the wrong one for deep creases. If one clinic quotes 64 units and another quotes 30 for the same face, ask each to explain the difference before comparing the totals.
Line two: why the same face costs twice as much in Manhattan
The per-unit rate is where geography and the injector’s credentials show up. RealSelf’s patient reports put the average treatment at about $798 in New York City and $675 in Miami, against $370 in Chicago and $367 in Dallas-Fort Worth. CareCredit’s state data tells the same story at a gentler slope.
| Where | Average Botox treatment | Source |
|---|---|---|
| California | $540 | CareCredit |
| Washington, D.C. | $525 | CareCredit |
| Hawaii | $510 | CareCredit |
| New York | $505 | CareCredit |
| Alabama | $390 | CareCredit |
| Arkansas and Mississippi | $385 | CareCredit |
| National average | $445 ($325 to $710 typical) | CareCredit |
Within a city, a board-certified dermatologist or plastic surgeon injecting personally tends to charge at the top of the local range, and a nurse injector at the same practice often less. One plastic surgery practice, for example, lists Botox at $15 a unit with the surgeon and $13 with its physician assistant. Clinics pay roughly $400 to $600 for a 100-unit vial, or about $4 to $6 a unit, according to industry pricing guides, so most of what you pay goes to the injector’s time, training and overhead.

The line no quote prints: how often you’ll be back
Botox typically lasts three to four months, so the honest price is the annual one. At our quote’s 64 units and $15 a unit, three visits a year is $2,880 and four is $3,840. Haute MD’s 2026 guide lands in the same place for typical patients: $1,200 to $3,600 a year at three to four sessions.
Two levers bring that number down. The first is fewer areas: frown lines alone, at 20 units, run $200 to $300 a visit by RealSelf’s estimate, or roughly $800 to $1,200 a year. The second is stretching the interval. Many patients find that once the muscles have been relaxed for a while, they can go longer between visits, though that varies widely and is a conversation to have with your injector rather than a promise.
Swap the brand name and the math changes
Botox is one of several FDA-approved botulinum toxins for frown lines, and the units are not interchangeable. Comparing per-unit prices across brands is the most common way shoppers fool themselves.
| Brand | How it prices out | How long it lasts (reported) |
|---|---|---|
| Botox | $10 to $15 a unit at most practices; 20 units for frown lines | About 3 to 4 months |
| Dysport | Takes about 2.5 to 3 units to match one Botox unit, so the per-unit price looks lower | 3 to 5 months; onset in 2 to 3 days |
| Xeomin | About the same dosing as Botox | Similar to Botox |
| Jeuveau | Usually 20% to 30% less per unit than Botox | Not consistently reported |
| Daxxify | One clinic lists $12 a unit; its frown-line dose is about 40 units, or about $480 | Median about 6 months |
Daxxify is the interesting one for the annual math. A $480 treatment twice a year is about $960, close to what three or four 20-unit Botox visits would cost, with fewer appointments. How long it lasts on your face is individual.
One newcomer to watch for 2027: Galderma’s Relfydess, already sold in more than 20 countries, received a Complete Response Letter from the FDA in June 2026 over manufacturing-site and testing issues, not safety or efficacy. Galderma said it plans to respond quickly. As of November 2026 it is not approved in the US and has no US price, and Allergan has announced no 2027 change to Botox pricing.
Area pricing, bargain units and four other lines to question
- A flat per-area price. It is not a scam, and it can be a good deal if you need a high dose. But if you need 12 units in an area priced as if it were 25, you are overpaying. Ask for the unit count either way.
- A per-unit price near wholesale. With clinics paying about $4 to $6 a unit, a rate far below the local market leaves little room for anything but cut corners. It is fair to ask which brand you are getting and to see the sealed vial.
- Injections outside a medical setting. In 2024 the CDC and FDA tracked harmful reactions to counterfeit or mishandled botulinum toxin in 19 people across nine states. All of them had been injected by unlicensed or untrained people in settings such as homes or spas.
- A Dysport or Jeuveau price compared directly with Botox. Convert the units first, or the cheaper-looking number may not be cheaper.
- An unexplained touch-up charge. Many practices include a check at about two weeks; others bill any extra units. Find out before the first visit, not at the second.
- Package prepay with no expiration terms. Buying three sessions up front can save money, but get the refund policy in writing in case you move or switch injectors.
Points, memberships and the discounts that do show up
Allergan’s Allē loyalty program is the most widely used discount on Botox itself. Under the terms in place since October 2024, according to a practice-software help center, each Botox treatment earns 200 points, up to four treatments a year, and 1,200 points can be redeemed for a free 50-unit vial at your practice. Members who earn 1,200 or more points a year can still redeem for cash. Allē also runs periodic dollar-off promotions in its app. Terms change, so confirm the current rules in the app before counting on them.
Practice memberships are the other route. IAPAM’s 2026 guide says membership plans typically bring the effective rate to $10 to $14 a unit in standard markets and $14 to $18 in big cities. A membership makes sense if you already go three or four times a year to the same practice. It makes less sense if you are still deciding if you like the results.
Questions patients ask after the first quote
How much is baby Botox or a lip flip?
Less than a standard treatment, because it uses fewer units. RealSelf’s patient-reported prices start around $181 for a lip flip or micro treatment, compared with a $586 average for a typical visit. You are still paying the same per-unit rate, so ask how many units the plan uses.
Does insurance pay for Botox?
Not for wrinkles. Cosmetic Botox is an out-of-pocket purchase. Some medical uses of the same drug, such as chronic migraine, may be covered by health insurance, usually with prior authorization. Ask your insurer and a licensed physician whether your situation qualifies.
How much is Botox per unit in New York, Los Angeles or Miami?
Expect more than the national figure. IAPAM’s 2026 pricing guide puts premium urban markets such as New York, Los Angeles and Miami at $18 to $35 per unit, against $10 to $20 in standard markets. RealSelf reports an average treatment of about $798 in New York City and $675 in Miami.
What does Botox for neck bands cost?
The FDA label for treating platysma bands calls for 26, 31 or 36 units depending on how many bands you have. At $10 to $15 a unit, that works out to roughly $260 to $540 per session before any upper-face work.
How many units of Botox do I need for my first visit?
It depends on the areas and your muscle strength. The FDA label doses are 20 units for frown lines, 24 for crow’s feet and 40 for forehead plus frown lines together. Many injectors start near those doses and adjust at a follow-up visit, so a first visit is a good time to ask how touch-ups are billed.
Reading your own $960 quote
Divide the total by the unit count. If the result sits between $10 and $15, or up to about $25 to $35 in a major coastal city, the price is ordinary, and the only question left is if you need every area on the list. If the unit count is missing, ask for it. If the per-unit figure is far below local rates, ask which product is in the syringe. And multiply by three or four before you decide, because that is the number that will actually leave your bank account in 2027. For anything medical, from a history of drooping eyelids to neuromuscular conditions, the right person to ask is a licensed physician, not a price list.
