Most gynecomastia surgery quotes are set at the consultation, once the surgeon knows whether the tissue is fat, gland or both.

Is it fat, or is it gland? A surgeon can usually tell within a minute of pressing on your chest, and that answer moves your gynecomastia surgery quote more than anything else. For 2027 planning, the American Society of Plastic Surgeons puts the surgeon’s fee alone at a projected $5,000 to $9,000, based on its 2024 member survey. Add anesthesia, the operating room and the compression vest, and RealSelf’s patient-reported average climbs to $9,115, with real bills running from about $5,000 to $16,000.

That is a wide spread for one operation, and most of it is explained by what is actually under your skin, how much skin there is, and where you live. Find your situation below and you will know which end of the range to expect. If you want a rough all-in number for your ZIP code first, our medical and dental cost estimator gives a starting point.

Start with what you see in the mirror

Only an exam confirms the diagnosis, but most men can place themselves in one of these rows before the consultation. Each row points to the section that prices it.

What you noticeWhat surgeons usually doWhere your quote tends to land
Soft fullness across the whole chest, no firm lumpLiposuction aloneLow end; shortest operation
A firm, rubbery disc behind one or both nipples (“puffy nipple”)Gland excision, usually plus liposuctionMiddle of the range
Sagging chest, nipples pointing down, loose skin after weight lossExcision with skin removalHigh end; one Philadelphia practice prices severe cases at $12,300 to $14,700
You are a teenager, or growth started with a medication or steroidsOften wait and treat the cause first$0 now; surgery later if it persists
You already had surgery and still see a bulge or a dentRevisionA fraction to most of the original fee

Soft all over: when liposuction alone is enough

Surgeons call fat-only enlargement pseudogynecomastia. There is no true breast gland to cut out, so the job is liposuction through a few small incisions, often under IV sedation rather than general anesthesia, according to ASPS. Less time in the operating room and a lighter anesthetic are why this version sits at the bottom of most price sheets.

The catch is that many men who think they have fat also have some gland, and liposuction does not reliably remove it. Dr. Miguel Delgado, a California surgeon who says he performs about 160 gynecomastia operations a year, writes that liposuction alone works only for pseudogynecomastia and is a leading reason he sees men for second operations. A cheap lipo-only quote is a good deal only if the diagnosis is right. If you are within reach of your goal weight, losing it first may shrink the job, and ASPS lists “relatively normal weight” among the traits of a good candidate.

Chest liposuction is priced like any other zone, so our liposuction cost guide is a useful cross-check if a surgeon quotes you for lipo only.

A firm disc under the nipple: the gland has to come out

This is true gynecomastia, and it is the case most gyno surgery prices are built around. The surgeon removes the gland through a small incision at the lower edge of the areola, then usually liposuctions the surrounding chest so the result blends. Expect general anesthesia or deep sedation, a compression garment for several weeks, and sometimes a thin drain for the first few days.

So how much is gyno surgery for a typical case like this? Published all-in prices collected by GoodRx show the shape of it:

  • Jacksonville, Florida: $4,999 to $7,499
  • Linwood, New Jersey: $6,000 to $9,000
  • Chicago: $6,000 to $12,000
  • Philadelphia: $8,600 to $9,200 for a typical case
  • Seattle: from $7,485
  • Dallas: $7,500 to $15,000

Ask which of these items the number covers, because GoodRx found that quotes may leave out anesthesia, the facility, the pre-op consult, tissue testing, prescriptions, garments or follow-up visits. A quote that looks $1,500 cheaper sometimes simply omits the anesthesiologist.

Sagging skin after weight loss: the priciest version

Once the chest droops, removing tissue is not enough; the extra skin has to go too, and the nipple may need to be moved higher. That means longer incisions, longer surgery and more visible scars. The Philadelphia practice in GoodRx’s survey shows the jump clearly: $8,600 to $9,200 for typical cases, $12,300 to $14,700 for severe ones. Men in this group are also the ones who most often push RealSelf’s range toward its $16,000 ceiling.

If you have lost a large amount of weight and also want work on the abdomen or arms, ask about combining procedures. Sharing one anesthesia and one facility booking usually costs less than two separate trips, though the total bill is still large. Our tummy tuck cost guide shows what that second procedure adds.

Teenagers, steroid users and men on certain medications: why waiting can be free

Breast growth during puberty often settles on its own, and ASPS notes that adolescents who have surgery may need another procedure later if development continues. That is why insurers and many surgeons want to see it persist first. Cigna, for example, looks for pubertal gynecomastia lasting at least two years before it will consider coverage.

The same logic applies when growth started with anabolic steroids, testosterone use or a prescription drug. ASPS lists “non-drug users” among good candidates, and Aetna’s policy says treatment should first aim at reversible causes, such as stopping the drug. Never stop a prescribed medication on your own; ask the doctor who prescribed it whether an alternative exists. Operating while the cause is still active risks paying twice.

Will insurance pay? Usually not, but the rules are written down

ASPS says that in most cases, correction of gynecomastia is not eligible for insurance coverage, and Aetna’s policy calls surgery or liposuction for gynecomastia cosmetic. Other insurers do cover it in narrow cases. Cigna’s coverage policy, effective August 15, 2026, is one of the clearest, and its next scheduled review is August 15, 2027, so check for changes if your surgery falls after that date.

Cigna’s checklist for medically necessary gynecomastia surgery
  • Klinefelter syndrome, or all of the following:
  • Gynecomastia that started in puberty and has lasted at least two years, or started after puberty and has lasted at least one year
  • True glandular tissue confirmed by exam or mammogram
  • ASPS Grade II, III or IV, documented with front and side photos
  • Persistent breast pain despite pain relievers
  • Any drugs or substances that can cause it identified and stopped for at least a year, when medically appropriate
  • Hormone problems ruled out with lab tests, or treated for at least 12 months first

Cigna does not cover surgery done to improve appearance or to address psychological distress alone, and it treats liposuction as the sole treatment as not medically necessary. If you think you qualify, your surgeon’s office will need to submit for prior authorization; get the approval in writing before you book. Medicare covers it only when medically necessary, and when it does, GoodRx estimated a patient’s share at about $375 at an ambulatory surgery center versus about $808 in a hospital outpatient department. Coverage varies by plan, so confirm with your insurer and a board-certified plastic surgeon.

The same documentation game applies to women seeking breast reduction, which is covered far more often. Our breast reduction cost guide compares the insured and cash prices.

Your ZIP code can move the bill by $5,000 or more

RealSelf’s state and metro averages come from patient reviews submitted through March 2023, so treat them as relative, not as next year’s sticker price. The pattern is what matters: coastal California costs roughly double the cheaper Midwest states.

Lower-cost marketsAverageHigher-cost marketsAverage
Indiana$3,563Orange County, CA$12,750
Florida$5,056San Francisco$11,663
Miami$5,444Los Angeles$10,920

Missouri$5,725Dallas-Fort Worth$8,081
Ohio$6,130New York City$8,050

Traveling a few hours can save real money, but price in the trip: a flight or drive, a hotel night or two, and a return for follow-up visits. Most surgeons want to see you within the first week, often to check or remove drains.

Already had surgery and still see a bulge or a dent

Second operations are more common than the before-and-after galleries suggest. A study Delgado cites found a 17.4% rate of second procedures among men who had gynecomastia surgery, rising to 34.8% for Grade II. The usual reasons are leftover gland behind the nipple, a “crater” where too much tissue was removed, uneven sides, or loose skin that was never addressed.

Revision is not cheap. Allure Esthetic in Seattle lists gynecomastia revision starting at $7,485. Delgado says his revisions cost around 25% of the original surgeon’s fee under local anesthesia and around 50% under general. Craters are harder to fix than leftovers, sometimes needing fat grafting. Surgeons generally want you to wait at least four to six months for swelling to settle before judging the result.

The cheapest revision is the one you avoid. Ask your first surgeon how many gynecomastia cases they do a year, whether they plan to remove gland or only liposuction, and what they charge if a touch-up is needed.

Gyno surgery questions men ask after the consult

Can gynecomastia come back after surgery?

It can, but it is uncommon when the gland is fully removed. Allure Esthetic in Seattle cites recurrence under 5% in that case. Weight gain, steroid use or a hormone problem can bring tissue back, which is why surgeons want stable weight and any cause treated first.

How much time off work does gynecomastia surgery need?

Delgado says most of his patients return to desk work within a week, but heavy lifting and hard workouts usually wait four to six weeks. If your job is physical, budget for unpaid time off on top of the surgery cost.

Do men think gynecomastia surgery was worth the cost?

Most do. RealSelf reports a 95% Worth It rating from patient reviewers, which is high for any cosmetic procedure. Unhappy reviews tend to involve leftover tissue or crater deformities, which is why choosing an experienced surgeon matters.

Can I finance gyno surgery?

Yes. ASPS notes that many plastic surgeons offer patient financing plans, and medical credit cards are common. Read the terms closely: deferred-interest offers can charge interest back to day one if you miss the payoff date.

Is the compression vest included in the price?

Sometimes. GoodRx lists post-surgery compression garments among items a quote may leave out. Ask whether the first garment is included and how many weeks you must wear it, since many men buy a second one to wash and rotate.

Get these three numbers in writing before you book

  1. The all-in price, itemized: surgeon, anesthesia, facility, garments, medications and follow-up visits. Anything missing is a future bill.
  2. The plan for your tissue: liposuction only, gland excision, or skin removal. If the surgeon plans lipo only and you feel a firm disc, ask why.
  3. The revision policy: what a touch-up costs if leftover tissue shows at six months, and whether the surgeon’s fee is waived.

Two or three consultations with board-certified plastic surgeons, compared on those three numbers, will tell you more than any average. Prices vary by surgeon and region, so confirm details with the practice and, if you hope to use insurance, with your plan.

Pricing method: prices were researched in November 2026 for 2027 planning and may change. Figures come from the American Society of Plastic Surgeons (2024 fee survey), RealSelf patient reviews, GoodRx’s survey of published practice prices, and the Cigna and Aetna coverage policies cited.
